Financial institutions—fine collection notices

If you receive a fine collection notice (FCN) for the redirection of a debt or for the regular redirection from a financial institution account, you—as the financial institution—must deduct the specified amount as noted on the FCN and pay that to the Registrar of SPER. It is also your responsibility to give written notice to the account holder of the amount deducted under the notice.

Use the following forms when dealing with us.

Return this form with remittance details for up to 2 debtors (include nil returns).

Note: A reason must be included for all nil returns. 
FCN Bank Return—Page 2 (Form 3021a) (PDF 325 K) Complete this form if you have more than 2 debtors (attach to Form 3021).
